As the facility for male offenders sentenced to solitary imprisonment and confinement at hard labor, the State Prison employed a physician appointed by the governor to attend inmates. Pursuant to St 1880, c 250, the prison physician, along with a physician appointed by the Board of Health, Lunacy, and Charity, was required to examine any inmate being considered for transfer to an asylum for the insane, to notify the warden about his finding, and send to a report to the governor, who made the commitment determination. Letterbooks document these activities.

The State Prison was opened in 1805 at Charlestown, Boston, as a successor to the prison on Castle Island. During 1878-1884 the prison was closed and inmates kept at Concord. With that exception, Charlestown remained the Massachusetts state prison until replaced by Massachusetts Correctional Institution, Walpole, 1955-1956.
